GDPR & PHOTOGRAPHY PERMISSION - I give permission for my child’s photo to appear on our website/publicity (players are never identified on social media and all FA safeguarding procedure regarding photography is followed). If you do not give permission, please also make your team manager aware. I accept that my contact details may need to be shared with third party photography partners. Also each group will have a WhatsApp group to communicate as a group training, matches and information on the time

BEHAVIOUR STANDARDS - I promise to abide by the Spectators code of conduct; My child promises to abide by the players code of conduct; I will not coach my child or give instructions from the sideline; I will be respectful at all times to referees, coaches, players, both from our club and the opposition; I will applaud good play and conduct from both teams; I understand that the coaches are volunteers and are giving up their time to provide footballing opportunities for my child. I will support them, and if I have any issues I will raise them privately.

If this is your first season at Mudeford Phoenix a £20 signing-on fee is to be paid and this will be followed with monthly payments of £20. If all members can set up a monthly standing order of £20 (per child) to Mudeford Phoenix GLFC starting in September to the following account:

If you are a new parent joining the club only - all players and parents will need to complete the East Christchurch Sports and Social Club membership online form - please can this be given to Darren Lewis with payment of £30. We will need the membership numbers for new registrations and we will renew this yearly as part of being with the club.
